Patient InformationAppointmentsIt is required for you to please bring all medications that your are currently taking for each and every appointment.Appointments may be scheduled by calling during regular office hours. Patients must always bring medications, immunization records, and past medical history. Every effort will be made to provide the earliest possible attention for the convenience of the patient. Likewise, If you are unable to keep your appointment, you must cancel two (2) hours prior to an appointment to avoid a fee. Please make sure you bring your insurance card(s) to all appointments. We realize that situations arise when you are not comfortable waiting until the next day to see a physician. Our goal is to meet your immediate medical needs, outside of regular business hours, in a timely and efficient manner. Through HPG at Night we will be seeing patients at our location from Monday through Friday, 5 p.m. to 11p.m., and on Saturdays and Sunday, from 3 p.m. to 11 p.m. Our physician will only see patients in need of immediate medical attention, with no appointments necessary. We ask that all prescription refills, physicals, regular appointment scheduling and referrals be addressed during our regular business hours. You will continue to have access to our "on call" physicians through our main office number of (609) 587-6661 as well. EmergenciesIn the event of a life-threatening, medical emergency, please call 911.A medical emergency is the sudden and unexpected onset of a serious condition. Examples are chest pain, severe shortness of breath, disorientation, severe illness, or loss of consciousness. If you go to the hospital in an emergency situation, you must notify our office at (609) 587-6661 before going so that we can obtain authorization from your insurance company. Hospital AffiliationWe are owned by Capital Health System (CHS) and utilize services at the health system.InsurancesHPG accepts most major insurancesPaymentsPlease bring your insurance card(s) with you to all appointments. Your payment must be received at the time of your appointment.PrescriptionsPlease call the office during regular office hours for a routine prescription refill at least two days before with your prescription name, medication, strength, dosage, pharmacy name and phone number, and instructions where to call you when ready.Our goal is to call in your refill by the end of our business day. Please do not call for prescription refills after office hours or during HPG at Night hours. ReferralsFor primary care patients, most referrals originate in our office and must be approved by one of our physicians. The doctor may ask to see you first before referring you to a specialist.We require at least 72 hours for non-emergency referrals. Please call the office with your request during regular office hours and have your insurance information available. After three business days you may pick up your referral. If a specialist requires any diagnostic tests, you may need to obtain a referral and authorization from our office. Have ready the name and phone number of the doctor, diagnosis and IDC-9 number of visits required, and where the procedure is taking place. Referrals are valid for only a specific period of time. Please check with your insurance company for specific instructions regarding coverage. Telephone CallsOur office staff is available during regular office hours. HPG at Night hours are not considered regular office hours.Routine needs such as test results, medical needs, referral requests or setting up appointments are handled most efficiently during this time when we are able to reference your chart personally. No refills or referrals will be issued on nights and weekends or during HPG at Night hours. |
